The Diabetes Control and Complications Trial (DCCT) was a clinical study conducted by the United States National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) that was published in the New England Journal of Medicine in 1993. Test subjects all had diabetes mellitus type 1 and were randomized to a tight glycemic arm and a control arm with the standard of care at the time; people were followed for an average of seven years, and people in the treatment had dramatically lower rates of diabetic complications. It was as a landmark study at the time, and significantly changed the management of all forms of diabetes.[86][130][131]

An unbalanced microbiome composition, known as dysbiosis, has been found in patients with diabetes, for whom the diversity of the gut microbiome is often reduced as compared to healthy people. Researchers from the University of Amsterdam recently showed that fecal transplants, used to transfer the microbiome of a healthy person to the gut of one with diabetes, can result in a short-term improvement of the insulin resistance found in obese patients with type 2 diabetes.

Certain drugs that affect the brain can help to control chronic pain. Antidepressants in the class of drugs known as selective serotonin reuptake inhibitors (SSRIs), which include duloxetine (Cymbalta), are often prescribed for pain. So are tricyclic antidepressants (TCAs) such as amitriptyline (Elavil). SSRIs often boost energy, while TCAs can make you sleepy. According to the Harvard Medical School website, “Some physicians prescribe an SSRI during the day and amitriptyline at bedtime for pain patients.”
In type 2 diabetes the body has an increasingly harder time to handle all the sugar in the blood. Large amounts of the blood sugar-lowering hormone insulin are produced, but it’s still not enough, as insulin sensitivity decreases. At the time of being diagnosed with type 2 diabetes, diabetics usually have ten times more insulin in their bodies than normal. As a side effect, this insulin stores fat and causes weight gain, something that has often been in progress for many years before the disease was diagnosed.
According to Jack D. Bragg, DO, associate professor of clinical medicine at the University of Missouri, most diabetes-related GI issues stem from a problem with one thing: motion. The digestive tract is a finely tuned muscular machine. It pulls and pushes, expands and contracts, relaxes and squeezes to carry food from your mouth, down the esophagus, into the stomach, and, finally, through 25 feet of nutrient- and water-extracting intestine that ends in, well, your exit.
Replacing humans with computers could make patients better control their sugar levels and suffer less complications in the long term. The French company Cellnovo has already shown that just a partially automated system, where blood sugar levels can be monitored wirelessly but patients still select insulin amounts, can reduce the chances of reaching life-threatening low sugar levels up to 39%. She says that the problem with diabetes is that it’s a silent disease. “Apart from needing to go to the loo a few times in the middle of the night, I experienced zero symptoms. This deluge of products alternately offered to fill attendees with energy or to calm us down, but almost never to keep us as we were. The implicit allure of such products was that we were not okay, or at least could be better. Given all the ways in which most people believe we could be improved, “wellness” has become an all-encompassing concept and industry that not only eats into the territory of mainstream medicine, but that has subsumed what used to be called “alternative medicine”—that which alludes to scientific claims when convenient and also defines itself in opposition to the scientific establishment.

Neuropathy is one of the common effects of diabetes. It’s estimated that 60-70 percent of people with diabetes will develop some sort of neuropathy throughout their lives. By 2050, it’s estimated that over 48 million people in the United States will be diagnosed with diabetes. That means in the future, anywhere from 28-33 million Americans could be affected by diabetic neuropathy.
When you have type 2 diabetes, your cells don't get enough glucose, which may cause you to lose weight. Also, if you are urinating more frequently because of uncontrolled diabetes, you may lose more calories and water, resulting in weight loss, says Daniel Einhorn, MD, medical director of the Scripps Whittier Diabetes Institute and clinical professor of medicine at the University of California in San Diego.
